The Tear Trough is the groove that runs under the eye between the lower eyelid and the cheeks which is commonly perceived as just skin discoloration and dark circles. In reality, the dark colour of the circles is often greatly exaggerated by a pronounced tear trough under the eye, as it will deflect light, thereby creating a shadow and an apparent increase of under eye circles and discoloration.

Under eye hollows may be hereditary and can occur at relatively young ages. Most commonly they occur as we age. As the fat directly under the eye begins to protrude with age, it can overhang and overshadow the tear trough, making it seem relatively deeper and darker and interrupting the previously smooth transition from the eyes to the cheeks.

Whatever the cause, prominent tear troughs or under eye hollows and dark circles contribute to a face that looks tired, prematurely aged or chronically ill. As the eyes are the centrepiece of the face and most commonly bear the first signs of ageing in the face, most people notice this “tired” look before anything else.

Recent advancements in minimally invasive aesthetic procedures have made it possible to significantly reduce dark under-eye circles that are caused by the tear trough deformity. Hyaluronic Acid (HA) injectable Dermal Fillers such as Juvederm, Restylane and Emervel (made of safe, natural sugar molecules that are already in our body, with a gel consistency) are perfect for layering in the tear trough and around the eye to give the lower lid a naturally rejuvenated look.

This “natural replacement therapy” effectively camouflages eye hollows and restores volume to create a smoother, more youthful transition from the eyes to the cheeks. Because dark circles are often the result of shadows thrown on the tear trough by overhanging fat, by lifting and filling the tear trough these shadows are eliminated.

Small droplets of the HA dermal filler are placed very deep along the Tear Trough and around the eye rim and massaged and sculpted into place. Around three to five injections are required on each side.

The Tear Trough Fillers have built-in local anaesthetic allowing for a quicker and more comfortable treatment of the patient as well as less swelling. Ice and numbing cream can also be used to numb the area.

The final results are evident at about a week after all swelling has subsided. Results under the eyes last for up to 12 months. Lumps are rare however, given the formulation of the product, can be readily dissolved.

What are the risks and downtime?
The dermal filler used is a natural skin component and is therefore very safe and well tolerated. Minor, temporary bruising is possible whenever needles are used in any skin procedure and can take 2-7 days to resolve. The extent of swelling is variable but it is most pronounced during the first week..We suggest that you take Arnica and Bromelin starting the day prior to the procedure and ask that all medications or supplements which can promote bleeding be stopped two weeks prior. Treatment should not be performed just before an important event.

Does it hurt?
Most patients find the procedure tolerably comfortable and require no anaesthesia. However, for those who are sensitive, numbing cream and/or ice can be applied beforehand to make the procedure more comfortable.

How long is the procedure?
It takes an average of 40 minutes

How long will the effect last?
The results last 6-12 months: the effect appears stable in most patients for the first 6 months and the loss is gradual after this. A good average re-application time to keep in mind is around one year although many patients have good effect well after one year.

What if I don’t like the results?
There is an antidote: an enzyme called hyaluronidase , that once injected will selectively and immediately dissolve the hyaluronic acid away.

Platelet Rich Plasma (PRP) Therapy is also a great, natural treatment for improving skin texture and elasticity under the eyes without using Tear Trough Fillers. It uses your own plasma as a seed to stimulate further production of collagen and naturally improve the look of your skin. For more information please see Platelet Rich Plasma (PRP) Therapy.
